Limited Time Game Modes (LTGM), formerly Limited Time Playlists (LTP) until late 2021, are temporary custom gamemodes in Insurgency: Sandstorm. They are named custom modes with custom rulesets known as "mutators", created by New World Interactive and made publicly available as Versus and Cooperative modes for a limited time (typically around a month, though in rare cases and special events they may last for shorter spans of time). The rulesets used by LTGMs are based on those available for custom online servers.

LTGMs were introduced in Update 1.4. Since January 22, 2024, LTGMs have been decided in a monthly vote by the Official Insurgency: Sandstorm Discord.

Bolt Actions Only No Drops "Dance to the beat of the bolt-action. Scavenging weapons off enemies is not allowed." Ditto, but players cannot pick up weapons and items, forcing them to use bolt-action weapons instead of relying on other types of weapons dropped by enemies. Checkpoint
Budget Antiquing "Hold off the hordes with guns from the back of the closet." Only a selection of old, outdated weaponry is available (e.g. M1 Garand, Grease Gun). Outpost
Chad Team 6 "Who you call when the alphas in alpha squad aren't cutting it. Fight more enemies with more supply." Each player has 100 Supply Points, allowing them to afford practically any loadout possible. This is balanced by a higher enemy spawn rate, meaning players are in more danger than usual. Checkpoint
Double Hardcore "Hardcore, but everyone dies in one shot." All characters are killed by any significant amount of damage, regardless of whether they have Armor on. Hardcore Checkpoint
Frenzy "Fight fast enemies who only attack with melee and Molotovs. Watch out for special enemies." All players are placed on Security and fight enemy Insurgents, who represent "zombies" and all only use the Khukuri. Special enemies include Flamers (throw Molotovs and drop one upon death), Jumpers (attempt to teleport behind players), and Bruisers (large enemies with a massive amount of health). Frenzy
Frenzy Headshots "Fight fast enemies who only attack with melee and Molotovs. Bullets can only deal damage to the head." Ditto, but enemies only take damage from headshots, per a common zombie trope. Frenzy
Guerillas "Capture and destroy objectives one at a time as you move through the area. Make do with fewer supply points." Players have less Supply Points that do not increase with completed objectives, forcing them to scavenge from enemies or rely on mostly bare weapons. Checkpoint
Hardcore Lite "Hardcore, but fewer calories. Experience Checkpoint mode with slower more tactical movement." Regular Checkpoint but with the slower movement of Hardcore Checkpoint. No other changes are known to be applied. Hardcore Checkpoint
Hot Potato Coop "Enemies drop grenades on death. Get a headshot to prevent their grenade from dropping." Enemies drop a live grenade upon dying, regardless of whether they had one or were using one, unless they are killed with a headshot. Checkpoint
Hot Potato Frenzy "Fight fast enemies who only attack with melee and Molotovs. Everyone who dies drops a live grenade." Frenzy but with the grenade drop mechanics of Hot Potato. Frenzy
Makattack "Start with a Makarov. Scavenge weapons from the enemy, and tap Use at supply crates to grab ammo." Players only have access to the Makarov in their loadout, forcing them to scavenge from enemies. Checkpoint
Pistols in Arms "Capture and destroy objectives one at a time as you move through the area using pistols." Players only have access to secondary weapons, forcing them to scavenge from enemies or rely solely on whatever sidearm they chose. Checkpoint
Quadruple Hardcore "Triple Hardcore, and then some." All characters are killed by any significant amount of damage, all players start off with low Supply Points that must be earned by completing objectives, and only shotguns are available in the loadout. Hardcore Checkpoint
Running in the 90s "Move fast, soak hits, don’t aim, and experience Sandstorm co-op with a late 90s FPS twist." Players cannot aim down sights, but move faster and have far higher health, simulating the gameplay of late 1990s first-person shooters such as Half-Life and Counter-Strike. Checkpoint
Task Force 666 "Fight the Frenzied as a tactical team of special forces vampires." Frenzy on night maps with Hardcore Checkpoint rules, with the added mechanic of allowing players to steal health from enemies by damaging them. Frenzy
Triple Hardcore "Hardcore, but everyone dies in one shot and you need to earn all your supply points." All characters are killed by any significant amount of damage. Additionally, all players start off with low Supply Points and must earn more by completing each objective. Hardcore Checkpoint
